[ACPT] Always Care Physical Therapy is locally owned and managed, and refers Licensed Physical Therapists to private clients in homes and other facilities, for specific assignments. We try to match your wants with our needs. You are not required to accept any assignment we offer you, however if you do accept an assignment we expect you to do the job well. There are NO FEES to sign up with ACPT. This is an ideal opportunity for “Physical Therapy - MOM’s” who can work while children are in school. ACPT invoices for all therapy services and remits to the therapist weekly, and does all of the collections and maintaining of records. ACPT Licensed PT’s are Independent Contractors, and will be furnished a Form 1099 at the end of the year. Our competitive rates vary for each specific assignment. Upon assignment we will agree upon your rate for it.

We also have a Master of Physical Therapy available to support you. Please understand that we may not have exactly what you want right now, but if you are not in our system we will not know what you want, and our needs change by the minute, the hour, the day, and the week. If we don't have you processed, we can not call you to offer you work! Also, the more flexible you are the more work we can offer to you.

You will need to have a Personal Interview and Orientation which we do partly in our office, followed up with a phone review with our chief therapist at a later date. After that one office visit, we can do almost everything by phone and mail. Print this out and use this as a check list, for all of your information needed, for the Always Care Physical Therapy Inc. [ACPT] Interview.

        c AUTOMOBILE: To get to assignments!
        c PHONE: and/or Pager (To contact you for assignments)
        c UNIFORMS/Lab Coat: To present a professional appearance.
        c NO RECORD or Bad Habits: We DO Criminal Records Checks, & Drug Testing
        c 1-2 Years PT Experience (PTA Experience Considered)

YOU MUST HAVE AND BRING TO INTERVIEW the following items for photocopying
        c DRIVERS LICENSE (Georgia),
        c SOCIAL SECURITY CARD,
        c AUTO/Vehicle INSURANCE CARD, (Georgia)
        c CURRENT GEORGIA PHYSICAL THERAPIST LICENSE,
        c Proof of Malpractice Coverage (If less than 30 days left, on your current policy,
            you must furnish proof of renewal submitted.
        c CURRENT CPR CARD,
        c TB (PPD) Test Results within last year, or Chest X-Ray Results if you have been previously exposed.
        c PT job history - Employers Names - COMPLETE Addresses, INCLUDING Zip Codes, Phone Numbers and NAMES of             Supervisors Now -
